LVN Wound Care Nurse - Full-Time

Position Overview

Vasona Creek Healthcare is seeking a compassionate and detail-oriented LVN Wound Care Nurse to join our clinical team. The ideal candidate has a strong background in wound assessment and treatment, a commitment to quality resident care, and the ability to work collaboratively with the interdisciplinary team.

Key Responsibilities

• Perform comprehensive wound assessments and document findings accurately
• Implement and monitor individualized wound care treatment plans
• Clean, dress, and manage a variety of acute and chronic wounds
• Monitor healing progress and report changes to the physician and care team
• Educate residents, families, and staff on wound prevention and care
• Ensure compliance with infection control, safety standards, and facility policies
• Maintain accurate and timely clinical documentation

Qualifications

• Current California LVN license in good standing
• Wound care experience preferred (long-term care or skilled nursing experience a plus)
• Strong clinical assessment and documentation skills
• Excellent communication and teamwork abilities
• Ability to work independently and manage time effectively
