We are looking for a self‑motivated, detail‑driven MDS Coordinator to join our care team on a part‑time basis, with the potential to grow into full‑time hours . If you are passionate about accuracy, advocacy, and person‑centered care, this role is for you.

✨ What We’re Looking For

Required Qualifications:

• Current LPN or RN license
• RAC‑certified (Resident Assessment Coordinator)
• Strong organization skills and the ability to work independently
• A commitment to regulatory compliance and high‑quality resident care

Preferred Qualifications:

• Infection Preventionist certification (or willingness to obtain)
• Experience with MDS assessments and care planning processes
• Willingness to take on additional responsibilities as needed to support the clinical team

✨ What You’ll Do

As our MDS Coordinator, you will:

• Lead and manage the Minimum Data Set (MDS) process with accuracy and efficiency
• Collaborate with interdisciplinary team members to ensure compliance and resident-centered outcomes
• Assist with Quality Measures, Care Planning, and Case Mix
• Take on additional clinical or administrative duties that support strong operational flow
• Uphold our standard of excellence, ensuring residents receive the thoughtful, individualized care they deserve
