Responsibilities
- Coordinate incoming referrals for home health processing and verify insurance coverage.
- Manage incoming calls from patients, family members, physicians, and disciplines; triage and route inquiries as needed.
- Ensure all personnel are informed of patient hospitalizations and follow up with patients admitted to other facilities.
- Coordinate small programs with limited budget/impact; optimize resources to meet schedules and goals; may serve as working supervisor for a team or work group.
- Apply clinical knowledge to perform a wide range of advanced intake activities and scheduling of referrals.
- Day shift coverage: Monday–Friday, 8:00 am–5:00 pm; no on-call duties.

Requirements
- Active California LVN license (LVN certification required)
- Minimum 1 year of experience in clinical intake coordination, scheduling, or a related setting
- Completion of an accredited LVN/LPN program
- Strong communication and organizational skills; ability to coordinate with multiple disciplines
- Ability to work in a home health/hospice environment and handle referral processing and scheduling
