**Job Title:** Home Health Aide and Personal Care Aide
**Job Overview:**
We are seeking compassionate and dedicated Home Health Aides and Personal Care Aides to join our team. As a vital member of our healthcare team, you will be responsible for supporting daily living activities, promoting patient independence, and ensuring a safe and healthy environment for those in your care.
**Key Responsibilities:**
- Provide personal care services, such as bathing, dressing, and grooming.
- Assist clients with mobility, exercise, and physical therapy activities as directed.
- Monitor clients' overall condition and report any significant changes to healthcare professionals.
- Perform basic housekeeping tasks, including laundry, meal preparation, and cleaning.
- Support clients in managing medication and attending scheduled appointments.
- Build positive relationships with clients and their families to provide emotional support and companionship.
- Maintain accurate records of patient services and care provided.
**Qualifications:**
- High school diploma or equivalent required.
- Certification as a nursing assistant (CNA) or home health aide (HHA) preferred.
- Experience in home care or a similar setting is advantageous.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and patient-centered approach to care.
- Valid driver’s license and reliable transportation may be required depending on client location.
